School Issued Laptop Request Form
School-supplied laptops are intended only for students whose families would find it a financial burden to purchase a new or used laptop for their child.
Students are expected to return school laptops and adapters (chargers) in clean, undamaged, working condition in June, or as soon as they are able to purchase a laptop of their own. Parents are responsible for the replacement cost of lost, stolen or damaged equipment.
Please print the form linked here, complete it and return it to the PMSS office. Once we have processed the form, the student will receive an email indicating when the laptop can be picked up from the Library.
