Parents/Guardians, every school year we ask for the following forms to be completed.
MANDATORY Forms: complete the following forms
-School Policies eForm (This will be submitted automatically once complete. A 'receipt' will be sent back to your email with a confirmation of completion of the e-Form.)
-Emergency Release this will be sent home with students by paper. Alternatively, you can find a fillable pdf below. Complete the information in its entirety, print and send this back to your child's classroom teacher.

-Student Verification Form-a new process will be introduced shortly to submit the Student Verification Form for the 2023/24 school year.
At any time of the school year, please make sure that you inform the school office if you change your home address or telephone numbers (home phone or cell phone). This updated information is valuable as it is used for attendance and emergency purposes.
-Medical Alert Form(s): If your child has a medical condition that the school needs to be aware of, please complete the applicable medical forms as detailed as possible. If you are completing the "Request for Administration of Medication" form, you will need a Physician's Note to accompany this form. *Print all the necessary forms and submit to the school office.*
