Should there be any changes, please print and make the changes on the form and return it to the school as soon as possible.β
Change of address-βββThe following documents are required in order for us to change your address. One of the following: - Long-Term Tenancy Agreement β showing name and address
- Property Purchase Agreement β showing name and address
- Income Tax Statement β showing name and Province of Residency
- Property Tax Statement β showing name and address
- Proof of Employment within the Community (For Canadians and Permanent Resident only).
AND, two of the following:
- Utility Bill
- BC Driver's License or Enhanced BC Driver's License
- BC ID
- BC Vehicle Registration
- Canadian Bank Account statement -showing name and address
- Canadian Credit Card Statement β showing name and address
- Proof of current school year membership in a local organization
βChange of name (student): Legal Name Change document from government Change of Custody/Contact Order: Legal Court Document Should there be no changes required, no further action is required.
| OPTIONAL forms: -Medical Alert Form(s): If your child has a medical condition that the school needs to be aware of, please complete the applicable medical forms as detailed as possible. If you are completing the "Request for Administration of Medication" form, you will need a Physician's Note to accompany this form. *Print all the necessary forms and submit to the school office.* β
