• Proof of citizenship for Parent and Child (Birth Certificate, Passport, Canadian Citizenship Card, Permanent Resident Card)
• Proof of residency in British Columbia (one of the following:  property purchase contract, rental agreement, income tax statement property tax statement) and two of the following:( Canadian Bank or Credit card statement, utility bill,BC drivers license,  BC Vehicle Registration, BC ID)
• Proof of guardianship: Child's birth certificate (long birth certificate showing parent's name/guardianship)
