POLICY & PERMISSION and REGISTRATION FORMS

Each school year, students and parents must complete mandatory forms:

POLICY & PERMISSION form (*online)
DAILY HEALTH CHECK (*online)
STUDENT INFORMATION VERIFICATION (paper)
EMERGENCY RELEASE forms (paper)

*The Policy & Permission forms are now completed through SchoolCashOnline. We ask that you register at sd43.schoolcashonline.com as early as possible to ensure you are able to complete these forms.

Paper copies of the Student Information Verification and Emergency Release forms will be sent home in September. These forms must be completed each year and returned to the school as early as possible.

MEDICAL ALERT FORMS (Optional)

Any student with a medical condition that may affect them while at school must complete a MEDICAL ALERT form. Other follow-up/associated medical forms may be required. (for example, if there are specific medical conditions like Diabetes or Anaphylaxis)
Any student who requires medications at school must have completed the Request for Administration of Medication form by a parent/guardian AND by a doctor. These forms can be accessed on our website at the following link:

VOLUNTEER FORMS & Police Information Check (Optional):

Parents who wish to volunteer for school functions must complete volunteer application forms (including Volunteer Application, Code of Conduct, Driver Application and Vehicle information forms as appropriate) and Police Information Checks (formerly known as Criminal Record Check). The volunteer application form and driver application form are now available to complete through sd43.schoolcashonline.com

Police Information Check:

Families with a PORT MOODY address - PIC forms are completed online through Port Moody Police. There is a $10 charge. Please click on the link below and select APPLY NOW to continue. Next, select Police Vulnerable Sector Check (PVS) for School District #43. You will need a copy of the Police Information Check request letter that allows parents/guardians to receive a PVS for the vulnerable sector. This letter needs to be uploaded when you are completing your PVS. Please contact the school office for a copy of the letter.

Families with a COQUITLAM address - are processed at a Coquitlam RCMP office. You will need a Coquitlam PIC form which can be requested via email through the link below, or you can pick up a copy at the School Office. The School also provides a Police Information Check Request Letter that allows parents/guardians to receive the PIC at a reduced rate. Please contact the school office for a copy of the letter.

REQUIRED FORMS – 1 FORM PER CHILD PER YEAR

Must be completed at the start of each school year. This form is submitted via KEV School Cash Online.

Must be completed at the start of each school year. This will be sent home in paper form at the beginning of the year, or you can Click here to fill out the Emergency Release form. Print and return it to your child's school.

Note: you can save this PDF on your home computer and edit it each school year as necessary...a real time saver!

OPTIONAL FORMS

To be completed once at each school level by parents who are planning to volunteer. This form is submitted via KEV School Cash Online.

Note: you will need to submit a Police Information Check (PIC) to your child's school.

Coquitlam resident Volunteers are eligible to have the PIC fee waived, with a signed letter of authorization from the school. Please contact the office for a copy of the 'Request for Police Information Check' letter. If you require further information regarding the PIC, please contact either Coquitlam RCMP (604-945-1550) or the Port Moody Police (604-461-3456). If you are a resident of Coquitlam, the Coquitlam RCMP requires the following form to be completed, in addition to the letter from the school (click on link): Coquitlam Police information check application form

To be completed each school year by volunteers who are planning to transport students during school field trips. This form is submitted via KEV School Cash Online.

NOTE: For driving, in addition to the Driver Application, you will need to submit a driver's abstract and a criminal record check to your child's school as well as complete a Volunteer Application form.

To be completed at the start of each school year, but only if your child has a medical concern. Print, fill and return form(s) to your child's school.

If your child has a medical condition the school needs to be aware of, please complete the medical forms below and return it to École Moody Middle School of the Arts as soon as possible, along with any medication.

Medical Alert form - For all students with a medical condition
Request for Administration of Medication - If medication must (or may) be administered at school - please note this form must be signed by a physician.
Action Plan (Diabetic or Anaphylaxis) - please complete the appropriate 'Action Plan' document.

	To be completed once at each school level by parents and non-parents who are planning to volunteer at our school. Note: you also need to drop off a criminal record check (CRC) at the school office. Contact the school to ensure your CRC is still valid.

	To be completed at the start of each school year by volunteers who are planning to transport students during school field trips. Note: you will also need to drop off a driver's abstract and a criminal record check at the school office and complete a Volunteer Application form (once per school level). 	To be completed at the start of each school year by staff or students planning to use privately owned devices (laptops, iPods, Blackberries, etc.) by connecting them to the district wireless or wired network.

	To be completed when your child is going to be away from school.
