September Start Up – Forms for ALL Parents to Complete by September 30th, 2023
It's that time of year again when parents need to ensure that all required forms are filled out. We don't want your child to miss out on opportunities at the school because the proper form was not filled out. As well, we need to make sure we have the most correct information for you.
The school district will require you to use 2 separate programs to fill out these forms:
1. MyEd Family Portal
2. Cash on Line.
1. Student Verification Form (MyEd)
This is the form that makes sure the school/school district has the most recent information on your child.
- Please go into the MyEd Family Portal. (Use a laptop or desktop computer – not your phone or tablet). We have attached a PDF titled “MyED Family Portal Information" that will help you.
- On the right hand side, you should see the published Student Verification Form
- Please read over the form. If all information is correct, there is no further action required from you.
- If changes are needed, then
- Print the form and make the necessary changes and bring the form back to the school. Please note that if your name, address, guardianship, medical alert or citizenship has changed, you will need to show documentation to support the change.
- If you do not have a printer, you can contact us at westwood@sd43.bc.ca for a printed copy. You will need to come to the school to pick up the printed copy from 8:00am – 3:30pm.
2. School District Policy Forms and More (Cash On Line)
You should have received an email yesterday requiring you to fill out some required forms for this year. It is important that you fill these forms out each year, including this year. Your child will not be able to go on walking fieldtrips, participate in school wide activities, and other fun activities until these forms are filled out. Please do so this weekend!
