This page contains various electronic forms designed to facilitate communication between parents and the school. It is also a green initiative whereby we are actively trying to eliminate excessive printing and saving a few trees in the process.

REQUIRED FORMS – 1 FORM PER CHILD PER YEAR

 
Must be completed at the start of each school year. This form is submitted via KEV School Cash Online.

Note: KEV School Cash Online is an eCommerce site and so it may appear that you need to pay a fee for our forms; you will see however that the cost is always $0.00

 

Must be completed at the start of each school year. Click here to fill out the Emergency Release form. Print and return it to your child's school.  

Note: you can save this PDF on your home computer and edit it each school year as necessary...a real time saver! 

OPTIONAL FORMS

 

To be completed once at each school level by parents who are planning to volunteer. This form is submitted via KEV School Cash Online.

 


You need to provide the School with a Criminal Record Check prior to completing this form.

Please request the Fee Waiver letter from the school you intend to Volunteer at prior to applying for the Criminal Record Check.

This Criminal Record Check can be obtained by contacting the RCMP detachment in your city of residence: Coquitlam RCMP (604-945-1550) or the Port Moody Police (604-461-3456).

Please note that applying online is the first step in the volunteering process. The school administration needs to approve all applicants to complete the process.



To be completed each school year by volunteers who are planning to transport students during school field trips. This form is submitted via KEV School Cash Online.

Note: you will need to submit both a driver's abstract and a criminal record check to your child's school as well as complete a Volunteer Application form.

Note: you also need to drop off a criminal record check (CRC) at the school office. Contact the school to ensure your CRC is still valid.

 1) Complete & electronically submit the Driver Application by clicking on the left icon. Be sure to re-submit this form if you end up with a new vehicle through the school year or if your insurance changes.

2) Submit an ICBC Driver's Abstract every year by calling ICBC at 1-800-950-1498 to request that your DRIVER'S ABSTRACT be faxed directly to our school at 604-937-8050.

3) Be sure you have submitted a Criminal Record Check.

Note: you will also need to complete a Volunteer Application form (once per school level).

Access to District WiFi

Privately Owned Devices form to be completed by staff or students planning to use privately owned devices (laptops, iPods, Blackberries, etc.) by connecting them to the district wireless or wired network. Please submit a separate form for each separate device.

SEE BELOW for How To Find the  Wireless (WiFi) MAC address on your device.

Please discuss with your child the appropriate level of care and responsibility you expect them to maintain with personal equipment brought to school.

NOTE: The security of privately owned equipment is the responsibility of the owner and the school cannot guarantee against or compensate for loss, damage, or theft.

Important: Once this form has been submitted, your device must be set it up for you. This process is NOT AUTOMATIC. Please check the schedule outside Room 202 to connect your device to the WiFi network. *Note: if you filled out this form in the past and your device is still able to connect, you do not need to complete this form again. You only need to fill out this form if you need your device setup.



‭(Hidden)‬ HOW TO FIND THE WiFi MAC ADDRESS

 

  iOS (iPod touch, iPad, iPhone):
  1. Go to Settings > General > About
  2. Scroll down until you see the WiFi Address
  3. Your WiFi MAC address follows this format: xx-xx-xx-xx-xx-xx

Android Tablet

To locate the MAC Address of your Android phone or Android tablet, follow these steps:

  1. On the Home screen, tap the Menu key and go to Settings.
  2. Scroll down and tap About Tablet then tap Status and then scroll down to view WiFi Mac address.

Android Phone

Depending on your device, attempt to follow one set of instructions listed below:

  1. On the Home screen, tap the Menu button and go to Settings.
  2. Tap About Phone.
  3. Tap Status.
  4. Scroll down to see your WiFi MAC address.

Or,

  1. On the Home screen, tap the Menu button and go to Settings.
  2. Tap About Phone.
  1. Tap Hardware information.
  1. Scroll down to see your WiFi MAC address.

Or,

  1. On the Home screen, press the Menu key and select Settings.
  1. Now select Wireless and network.
  1. Select WiFi Settings. You will find the MAC address under the Advanced section.

Windows Computer

1. Click on start

2. Type “cmd”into the box at the bottom that says “search programs and files”.

3. Type “getmac –v –fo list”

4. The Ethernet MAC address will be listed next to the words “Local Area Connection” in the form xx-xx-xx-xx-xx-xx

5. The Wireless MAC address will be listed next to the words “Wireless Network Connection” in the form xx-xx-xx-xx-xx-xx

Apple Computer

1. Click on the apple on the top left corner

2. Click on About This Mac

3. Click on More Info

4. On the Side bar Under Networks, click on Locations

5. The Ethernet MAC address will be listed next to the words “MAC Address Ethernet” in the form xx-xx-xx-xx-xx-xx

6. The Wireless MAC address will be listed next to the words “MAC Address Airport” in the form xx-xx-xx-xx-xx-xx
