This page contains various electronic forms designed to facilitate communication between parents and the school. It is also a green initiative whereby we are actively trying to eliminate excessive printing and saving a few trees in the process. 

The following forms are available in your child's school agenda.

 Policies and Permission Forms - This form should be completed in September each school year. It ensures that parents and their child/children have read the school code of conduct as well as the district code of conduct and that parents are consenting to have their children participate in various school activities or programs.

Volunteer Application Form - This form should be completed by parents who are planning to volunteer at our school. Please note that you need to drop off a criminal record check at the school office.  

Volunteer Driver Application Form - This form should be completed by parents who are planning to transport children during school field trips. Please note that you will need to drop off a driver's abstract and a criminal record check at the school office.
