Get Active Card for Grade 5 students

Remember to bring the following documentation to receive your Grade 5 Get Active! Card: • A letter of verification from School District #43 identifying them as a Grade 5 student - students obtain this from their school's administration office not their teacher • Birth certificate or Care Card showing the student's age as of December 31st, 2012. • Proof of Coquitlam residency Bring the above documentation to the following locations: • Poirier Sport & Leisure Complex 633 Poirier Street • City Centre Aquatic Complex 1210 Pinetree Way • Pinetree Community Centre 1260 Pinetree Way
