POLICY & PERMISSION and REGISTRATION FORMS
Each school year, students and parents must complete mandatory forms:
- POLICY & PERMISSION form (*online)
- STUDENT INFORMATION VERIFICATION (paper)
- EMERGENCY RELEASE forms (paper)
*The Policy & Permission forms are now completed through SchoolCashOnline. We ask that you register at sd43.schoolcashonline.com as early as possible to ensure you are able to complete these forms.
Paper copies of the Student Information Verification and Emergency Release forms will be sent home in September. These forms must be completed each year and returned to the school as early as possible.
MEDICAL ALERT FORMS (Optional)
- Any student with a medical condition that may affect them while at school must complete a MEDICAL ALERT form. Other follow-up/associated medical forms may be required. (for example, if there are specific medical conditions like Diabetes or Anaphylaxis)
- Any student who requires medications at school must have completed the Request for Administration of Medication form by a parent/guardian AND by a doctor.
VOLUNTEER FORMS & Police Information Check (Optional):
Parents who wish to volunteer for school functions must complete volunteer application forms (including Volunteer Application, Code of Conduct, Driver Application and Vehicle information forms as appropriate) and Police Information Checks (formerly known as Criminal Record Check). The volunteer application form and driver application form are now available to complete through sd43.schoolcashonline.com
Police Information Check:
- Families with a PORT MOODY address - PIC forms are completed online through Port Moody Police. There is a $10 charge. Please click on the link below and select APPLY NOW to continue. Next, select Police Vulnerable Sector Check (PVS) for School District #43. You will need a copy of the Police Information Check request letter that allows parents/guardians to receive a PVS for the vulnerable sector. This letter needs to be uploaded when you are completing your PVS. Please contact the school office for a copy of the letter.
https://portmoodypolice.ca/public-info/police-information-checks/ - Families with a COQUITLAM address - are processed at a Coquitlam RCMP office. You will need a Coquitlam PIC form which can be requested via email through the link below, or you can pick up a copy at the School Office. The School also provides a Police Information Check Request Letter that allows parents/guardians to receive the PIC at a reduced rate. Please contact the school office for a copy of the letter.

REQUIRED FORMS – 1 FORM PER CHILD PER YEAR

Must be completed at the start of each school year. This form is submitted via KEV School Cash Online.

Must be completed at the start of each school year. Click here to fill out the Emergency Release form. Print and return it to your child's school.

OPTIONAL FORMS

To be completed once at each school level by parents who are planning to volunteer. This form is submitted via KEV School Cash Online.
Note: you will need to submit a criminal record check (CRC) to your child's school.
Please request the Fee Waiver letter from the school you intend to Volunteer at prior to applying for the Criminal Record Check.
This Criminal Record Check can be obtained by contacting the RCMP detachment in your city of residence: Coquitlam RCMP (604-945-1550) or the Port Moody Police (604-461-3456).
Please note that applying online is the first step in the volunteering process. The school administration needs to approve all applicants to complete the process.
To be completed each school year by volunteers who are planning to transport students during school field trips. This form is submitted via KEV School Cash Online.
Note: you will need to submit both a driver's abstract and a criminal record check to your child's school as well as complete a Volunteer Application form.

To be completed at the start of each school year but only if your child has a medical concern. Print, fill and return form(s) to your child's school.
