The BC Centre of Disease Control has just revised the Daily Health Check form.  You recently completed a previous version of the Daily Health Check, however the previous form has now been updated as the BC Centre for Disease Control has changed the list of symptoms and procedures that families need to follow. 

The new form is attached to this e-mail in English and several other languages.  Please read this immediately so you are aware of the health requirements.   

We will be asking you to acknowledge the new Daily Health Check form once it is uploaded to School Cash Online which will be accessible through our school website. I will send a separate email to let you know when the system is ready for you to log in to your account and submit the acknowledgement form.

Please note: you only need to do this once.  There is no need for you to submit the form on a daily basis.
