OPTIONAL FORMS
	To be completed at the start of each school year but only if your child has a medical concern.
	To be completed once at each school level by anyone planning to volunteer at our school. Note: you also need to drop off a criminal record check (CRC) at the school office. Contact the school to ensure your CRC is still valid, or to request a referral form for a volunteer fee waiver. A CRC can be obtained by contacting the RCMP detachment in your city of residence: Coquitlam RCMP (604-945-1550) or the Port Moody Police (604-461-3456).
	To be completed at the start of each school year by volunteers who are planning to transport students during school field trips. Note: you will also need to drop off a driver's abstract and a criminal record check at the school office and complete a Volunteer Application form (once per school level).For your driver's abstract, please call 1-800-950-1498 (ICBC Customer Service) and ask them to e-mail the document directly to LRichmond@sd43.bc.ca .
	The Privately Owned Devices form is generally for Eagle Ridge staff only. If your child needs network access for their educational program, please contact our principal Mrs. Reynolds. To be completed at the start of each school year by staff or students planning to use privately owned devices (laptops, iPods, Blackberries, etc.) by connecting them to the district wireless or wired network.
