At Port Moody Secondary School the Graduation Transitions Program is a four year program that incorporates the following:

1. DPA or Daily Physical Activity. This was mandated by the ministry in 2008, that All students are required to document 150 minutes of exercise per week.
2. HCE 9 or Health and Career Education 9. This program provides students with the knowledge, skills and attitudes that will assist them in making informed decisions about topics such as health, education, and future careers.
3. Career Exploration. All students are required to examine a career choice each year.
4. Graduation Transitions (GT) 12 is a self-paced course that is intended to prepare students for a successful transition to life after secondary school. It is a 4 credit requirement for Graduation. At Port Moody Secondary this program is delivered to students through Phoenix Advisory class and online. There are six activities that must be completed to receive credit for this course. The Six activities are:
1. Daily Physical Activity
2. Transition Plan
3. 30 hours of work or volunteer experience
4. Financial Plan
5. Resume and cover letter
6. Exit Interview

The Graduation Transition requirements are to be dropped off in the Graduation Transition office in room 3055 by the first week of February.  If the student is graduating in January, these documents are to be submitted by the last week of November.
